Hospital care at home

We understand that reducing the length of a hospital stay can be beneficial to our members' health and wellbeing. Through the Hospital Care at Home program, and under the guidance of their medical team, members can receive hospital care at home. Services may include wound management, medication management, home nursing and more.

These services are hospital substitute services.  This means that they are delivered safely and under the direction of your hospital treating team but in the comfort of your own home.  They are services that, if you were not receiving them at home you would have to remain in hospital to receive them. 

To be eligible for these services you must have hospital cover and have served all wait periods.

This program is brought to members through Teachers Healthcare Services, an initiative of Teachers Health to support our members' health and wellbeing through quality, evidence-based health management programs and services. To be eligible for this program you must be a member of Teachers Health and have Hospital Cover with all waiting periods served. Additional eligibility criteria may apply. Eligible members will then be connected with a dedicated Teachers Healthcare Services care coordinator who will support members on their personal journey.
