Diabetes management

In Australia, someone is diagnosed with diabetes every five minutes and just as many people could be living with undiagnosed diabetes. We understand that living with a chronic condition such as diabetes can be difficult but it can be made easier with support.

The Diabetes Management Program is designed to help members with either type 1, type 2, gestational diabetes or pre-diabetes to manage their health and learn the lifestyle skills which will support them in living the best life possible. Advice and support from a credentialed diabetes educator, an accredited practising dietitian and an accredited exercise physiologist are available to empower members participating in the program to adopt a healthy lifestyle and avoid, or minimise, potential short and long-term complications.

This program is brought to members through Teachers Healthcare Services, an initiative of Teachers Health Fund to support their health and wellbeing through quality, evidence-based health management programs and services. To be eligible for this program you must be a member of Teachers Health Fund and have Hospital Cover, with all waiting periods served. Additional eligibility criteria may apply. To find out more, contact Teachers Health Fund on info@teachershealth.com.au or call 1300 727 538. Eligible members will then be connected with a dedicated Teachers Healthcare Services care coordinator who will support members on their personal journey.
